Our philosophy is to deliver compassionate medical care in the home, utilizing a holistic approach to patient care. A personalized treatment plan is developed for each patient, encompassing the whole person including medical, physical, emotional, psychological, and spiritual needs. Informed patients and families are viewed as partners in care. We believe the patient has a right to make autonomous choices. It is our duty to uphold the rights of the patient, and show “ethical sensitivity” in dealing with moral dilemmas. We will provide primary care to those who are homebound, elderly, disabled, insured, and uninsured, and to any individual over the age of twelve, in need of health care. We will be an “active listener” and incorporate healing touch in every patient’s care. We will utilize an interdisciplinary approach in the treatment plan, and help to coordinate all services. We will follow guidelines for preventative medicine and promote healthy life-style choices. Education and guidance are essential components in our philosophy. An educated patient makes the best choices congruent with their own beliefs, the provider, as well as the family unit.
Through a collaborative approach with various disciplines from the health care team, we will strive to improve patient outcomes. The goal is to have the patient achieve a “quality of life” which aligns with the patient’s own values and beliefs.
It is our philosophy to foster a relationship of mutual respect based on cultural diversity of both patient and family. We will respond to concerns and issues, within a timely fashion, always by the end of the day. We will show respect for human dignity, and provide competent medical care, through honest interaction, and develop a relationship based on mutual trust.
